JOB SUMMARY

This position is responsible for day-to-day scheduling of staff and coordinating Elders In Home Care Services, to ensure overall goals and objectives are achieved to provide efficient, culturally sensitive, and quality home care to elders by designated staff and service providers.

MAJOR TASKS AND RESPONSIBILITIES

  1. Establish and maintain confidential and positive caring relationships with managers, staff, physicians, non-physician providers, departments, and elders/families.
  2. Evaluate staffing needs and makes recommendations as needed. Ensure adequate staffing levels for all shifts, to include coverage for planned or unplanned absences.
  3. Liaison with partnering staff agencies - working together to ensure all parties have adequate coverage.
  4. Provide support to Caregiving staff; assist with schedules of caregivers to ensure that needs of Elder’s are met.
  5. Coordinate with the Nursing staff to: Select, schedule and coordinate caregivers based on care plan information for current and new clients.
  6. Facilitate the interview process for caregivers.
  7. Review and effectively communicate the requirements of the care plan to potential caregivers; recognize training gaps and provide direction to training materials ensuring caregiver and client safety.
  8. Ensure EVV (Electronic Visit Verification) compliance by regulating and updating schedules for caregivers.
  9. Track and document any client/staff complaints and review with MEIHSS Leadership.
  10. Evaluate staff, address issues appropriately and continuously monitor staff to improve services provided.
  11. Familiar with concepts, practices and procedures of home care industry.
  12. Work productively and effectively within a complex environment, with multiple/changing priorities, support implementation of programs and procedures developed at an organizational level.
  13. Serve as a highly engaged and full partner on the care team and respond willingly to care team member needs for assistance and partnership. Participate in work that improves patient care and the professional practice environment through quality assurance surveys and phone inquiries.
  14. Develop and maintain a confidential client filing system, case records and required forms, including: service episode records, care plans, risk assessments and/or safety plans and ensures other documentation is obtained and filed appropriately. Create and maintain forms necessary for completion of scheduler’s task.
  15. Participate in a variety of meetings, workshops, and committees for the purpose of conveying and/or gathering information required to perform functions and maintaining professional knowledge.
  16. Collaborate with community leaders, professional staff/consultants, and elders/families to identify, define, and meet elder home care needs, goals, and objectives.
  17. Because of the Tribe's commitment to community service and the well-being of its members, each employee may be expected to perform a wide range of office and field duties from time to time. Such duties may or may not be related to their regular responsibilities.

EDUCATION - EXPERIENCE AND TRAINING FOR POSITION

Required:

  • High School diploma or GED
  • Current Driver’s license in good standing
  • Three (3) years of experience providing complex administrative support that includes at least one (1) year experience in health care environment

OR

  • High School diploma or GED
  • Current Driver’s license in good standing
  • NAC/HCA certification
  • Two (2) years administrative or staffing experience.

Preferred:

  • Experience working within a Tribal community.
  • Experience working with elderly.

 

SPECIFIC SKILLS/KNOWLEDGE/ABILITIES REQUIRED FOR POSITION

Skills:

  • Demonstrated skill in effectively interacting with diverse populations; communicating clearly and professionally both verbally and in writing; applying logic and sound reasoning to problem-solving and decision-making; and planning, organizing, and managing projects. Must be proficient in preparing and maintaining accurate records, using standard office equipment and personal computers, including relevant software applications, and managing time efficiently. Ability to address and resolve conflict in a calm, professional, and solutions-focused manner is required.

  • Demonstrate professionalism by maintaining strict confidentiality, a positive work ethic, and effective written and verbal communication. Employee must accurately comprehend and interpret information, gather and analyze data, exercise sound independent judgment, and present clear findings and recommendations. Knowledge:

  • Utilization review, quality assurance, and managed health care concepts; clinical and/or client assessment principles; elder care and in-home care practices; and HIPAA compliance requirements. Must possess proficiency in the English language, including proper grammar and written composition, and an understanding of human behavior. Knowledge of Native American culture and history is required to support culturally respectful, appropriate, and effective service delivery.

 

PHYSICAL REQUIREMENTS

This position requires the ability to remain alert and responsive for extended periods, including long shifts of up to 24 hours when scheduled of On-Call availability. The Staffing Coordinator must be able to continuously answer and monitor phone calls to manage caregiver call-outs and staffing needs. Duties require sustained attention, frequent use of a telephone and computer, the ability to respond promptly to urgent staffing situations, and the capacity to work irregular hours, including nights, weekends, and holidays as operational needs require.  The employee is continuously required to sit, talk, hear and use hands to finger, handle, or feel objects, tools or controls. The employee is occasionally required to drive a motor vehicle. The employee is occasionally required to stand, walk, climb, bend at the waist, kneel, grasp objects with the fingers, and reach with hands and arms.  The employee must occasionally push/pull up to 10 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus. The noise level in the work environment is usually moderately quiet.
