JOB SUMMARY

Responsible for providing home care patients with a full range of personalized care, which may include such items as instruction on chronic disease management and medication, pain management, wound care, IV infusion, and post-operative instruction.  Coordinates and supervise LPN’s and assigned home care providers in the delivery of patient care.

MAJOR TASKS AND RESPONSIBILITIES

  1. Provides skilled nursing service in accordance with the orders of a physician and under a plan of care in a patient’s home. Has the responsibility for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others.
  2. Records the health needs of the patient.
  3. Performs and completes start-of-care assessment, medication profile, initiates Nursing or Home Health Aide plan of care,
  4. Evaluates the effects of the care given and regularly re-evaluates the patient’s nursing needs on a regular basis.
  5. Evaluates and revises the plan of care, as necessary.
  6. Provides nursing services, treatments, and diagnostic procedures requiring specialized skills and assuming full responsibility for each patient.
  7. Initiates preventative and rehabilitative nursing procedures as appropriate for the patient’s care and safety. Effecting changes in environment to ensure patient safety and security.
  8. Counsels the patient and his/her family in meeting nursing and related needs.
  9. Coordinates services for patients as needed to other agencies and health care providers as appropriate
  10. Provides reports to the patient’s physician about patient under care when the patient’s condition changes or there are deviations from the plan of care.
  11. Provides supervision of LPN’s and home care providers.
  12. Prepares clinical and progress notes on the relevant and specialized nursing services provided and maintains a clinical record for each patient receiving care.
  13. Updates knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually.
  14. Adheres to applicable regulations, policies and procedures.
  15. Maintains the privacy and confidentiality of all protected health information in accordance with the divisions policies and procedures, and in compliance with state and federal regulations as applicable.
  16. Because of the Tribe’s commitment to community service and the well being of its members, each employee may be expected to perform a wide range of office and field duties from time to time.  Such duties may or may not be related to their regular responsibilities.

EDUCATION - EXPERIENCE AND TRAINING FOR POSITION

Required: Graduate of an accredited Registered Nurse (RN) Program and two (2) year RN experience in a home health/hospice, skilled nursing or acute setting.

 

Additional Requirements:  Current Negative TB Test; current in all immunizations; pass criminal history check; pass drug screening.

SPECIFIC SKILLS/KNOWLEDGE/ABILITIES REQUIRED FOR POSITION

Skilled in:  Communicating effectively in both verbal and written form; adapting to changing situations; assessing patients and monitoring performance; comprehending and interpreting records/documents; listening effectively; problem solving; troubleshooting; time management; multi-tasking; supervising staff.

 

Knowledge of: Practical nursing techniques and methods; methods used in implementing resident care program objectives; medications and treatments used in patient care; infection control procedures; mental or behavioral characteristics of physically or mentally disabled people.

 

Ability to: Follow verbal and written instructions; observe and record information used to make clinical and program staff decisions; observe and evaluate conditions, reactions and changes in the physical and mental conditions of patients; maintain appropriate attitudes and conduct necessary to the welfare of patients; maintain records and prepare reports; communicate effectively; maintain confidentiality.

PHYSICAL REQUIREMENTS

While performing the duties of this job, the employee is regularly required to stand and talk or hear. The employee is frequently required to walk; use hands to finger, handle, or feel; and reach forward with hands and arms. The employee is occasionally required to sit and stoop, kneel, or crouch. The employee must frequently lift and/or move up to 50 pounds.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. The noise level in the work environment is usually moderately quiet. May face hazards from exposure to chemicals and infectious diseases. May treat patients that are confused, irrational, agitated, and/or uncooperative.

Licenses or Certifications Required

Current and valid Washington State Registered Nurse licensure. A current and valid Washington State driver’s license. A current CPR or BLS Certification.
